Application Requirements

FRHS is legally required to take safety measures when protecting the privacy of patient information per the Health Insurance Portability and Accountability Act of 1996 (HIPAA). You will be expected to maintain patient privacy and confidentiality during and after your visit. Failure to do so can result in immediate termination of your job shadow and may result in civil or criminal penalties.

Patient’s protected health information (PHI) includes:

  • Patient information and records
  • Employee files, digital information, business records, photographs
  • Conversations between patient and employee or between coworkers
  • All events and records pertaining to patients, employees and hospital business

Waiver

When you sign and submit the application, you acknowledge:

  • I understand that information I see and hear about patient identity and conditions is considered confidential and is not to be discussed outside of my job shadow experience. Any violation of patient or hospital confidentiality will result in dismissal from the experience.
  • FRHS will take immediate action in any situation in which my behavior or performance adversely affects the best interests of the facility or patients. This action may include, but is not limited to, my removal from the facility and the experience.
  • FRHS is not responsible for lost or stolen personal belongings.
  • I will hold FRHS and its employees harmless in the event of incident, injury or illness. FRHS is not responsible for costs involved with emergency or follow-up care.
  • I will call the facility and cancel, in advance, my shadow experience if I suspect I may be ill.

Your Safety

All observers must wash their hands upon entering and exiting a patient’s room. If you have any active signs of illness, you must reschedule your job shadow. Depending on the professional you’re shadowing, you may be required to take additional safety precautions by wearing the protective equipment.

FRHS also requires you to provide a proof of a current flu shot if you’re visiting between Oct. 1–March 31.

Dress Code

Since patients may see you as an FRHS employee, we ask that you behave professionally. Please follow our rules on dress code.

Not allowed:

  • Jeans, sweatpants, leggings or shorts
  • Tank tops, sleeveless tops, spaghetti straps
  • Sweatshirts, hoodies
  • Open-toed shoes
  • Heavy perfume or scents
  • Hats

Skirts are allowed if they are no shorter than two inches above your knees. You must wear socks or hosiery with acceptable shoes. If you have long hair, we ask that you please wear it held back or up.
