Freeman Regional Health Services is proud to continue providing assisted living services in new locations designed to meet the changing needs of our community.
Assisted living services are now available at:
Walnut & Dewald Street Villages
A comfortable residential setting where residents can enjoy the independence of apartment living while having access to the support and care they need.
Oakview Terrace
A dedicated assisted living wing within Freeman Regional Nursing Home provides another option for residents whose needs are best met in a setting with additional support and access to 24-hour nursing care.
Continuing a Tradition of Care
These assisted living services have transitioned from the former Salem Mennonite Home location to new settings within Freeman Regional Health Services.
While the location has changed, our commitment to compassionate, personalized care remains the same. Familiar assisted living team members continue providing care to residents, helping make this transition as comfortable and seamless as possible.
Our goal is to support each resident’s independence, dignity, safety and quality of life while providing the assistance they need to continue calling Freeman home.
Assisted Living Services
Freeman Regional Assisted Living is licensed by the South Dakota Department of Health as an Assisted Living facility able to provide:
- Personal Care Assistance including dressing, grooming and hygiene support, incontinence management, bathing assistance (whirlpool available), and support with supplemental oxygen,
- Health Maintenance including 24 hour on-site presence of a nursing team member and regular observation and assessment of physical and cognitive health,
- Medication Management including medication administration and/or supervision available 24 hour per day,
- Food & Nutrition Services including three home-cooked meals daily, snacks, and therapeutic diets (as needed) under the direction of a licensed dietitian,
- Housekeeping & Laundry
- Activities & Socialization including fitness & exercise groups, socials, games, music programs, participation in community activities, chapel services, and more, and
- Transportation is available through the Freeman Transit at no additional cost within a 75 mile radius of Freeman.
- Medication administration
- Support for cognitive impairments
- Support for physical impairments
- Dependent on supplemental oxygen
Our assisted living services are designed to meet each resident where they are and provide the right level of support as their needs change.

By expanding assisted living services to the Walnut and Dewald Street Villages and Oakview Terrace, Freeman Regional Health Services is creating additional opportunities for residents to remain close to their community, friends, families and familiar caregivers.
