For a patient whose condition requires hospitalization, Freeman Medical Center is staffed with highly trained and compassionate providers offering a superior quality of care.
We are able to treat a wide variety of conditions and the hospital has comfortable private rooms. A broad array of support services are available on-site including Laboratory, Radiology, Pharmacy, Respiratory and Wound Care services.
You will receive individualized care, excellent support and post-admission planning with assistance, in a warm family atmosphere. Every member of our staff is committed to your comfort and care as we assist you on the road to recovery (and home) as soon as possible. We invite you to experience our facilities and capabilities. For our patients’ comfort, telephone, TV and wireless Internet service are provided free of charge.
Every patient is important, and your comfort is our concern
At Freeman Regional Health Services, we utilize a team approach to all patient care. We utilize several different departments to provide patient care planning. This is sometimes known as Care Coordination or Case Management and it is all about you. We coordinate your admission, your treatment plan, and your discharge plan in every care setting whether it be as an inpatient, as an outpatient, or in the Emergency Department.
Planning your treatment and discharge
Your Team will:
- Personally interview you on arrival.
- Keep in contact with you throughout your treatment to give you and your family the information you will need to participate in your health care decisions.
- Assess and coordinate your post-discharge needs.
- Provide consistent and coordinated care.
- Work with you, your family, and your insurance provider to facilitate a smooth transition of care after discharge.
- Arrange for post-discharge interventions which may include home health care, special visiting nurse care, supplies, equipment, and transportation.
- Help you use the Language Line to facilitate communications with FRHS providers and staff, and to share medical knowledge with your family.
